REGAL FAMILY PRACTICE MEDICAL CENTER welcomes you to our customized Immigration Medical Examination required by the USCIS.

We will help you complete all the requirements of Form I-693.  Call 310-533-9233 to make an appointment. 

REQUIREMENTS:

• Official Photo ID:  Passport, US Driver’s License, State ID)
• Immunization Record if Available

SERVICES / FEES:

1. Complete Examination
   1. $160 Age 1 to 14 years old
   2. $180 15 years and older
      INCLUDES: 
      • Detailed Medical History
      • Physical Examination & Mental Status Evaluation by a Certified Civil Surgeon
      • Verification & Completion of USCIS Required Immunizations
      • Quick Completion of Form I-693
      • Sealed Envelope containing Form I-693 with other pertinent medical results
      • Why wait? Completed form is available after 48 hours or on second visit if vaccination is complete, and if TB and Syphilis tests are negative.  If exam is done on a Saturday, completed form will be available on Tuesday.
2. $20 Blood Testing for Syphilis (required for 15 years & older)
3. $20 TB Skin Testing (Required for 2 years and older)
4. Vaccination (Type of vaccine to be determined by Civil Surgeon)  maybe done at your Private Physician's Office or at the TORRANCE HEALTH CENTER  FREE OF CHARGE

                                     FINAL STEPS:
When your entire test and physical exam is completed, you will receive:

• A sealed envelope containing all the appropriate forms filled out and signed.
  Note:  this must be presented with the original seal to the immigration officer in order to be considered valid.
• A copy of your immunization record and x-ray report (if applicable) for your personal records.
